When and how much CARBAGLU should I take?

A doctor who is experienced in treating metabolic disorders will start you or your child’s CARBAGLU® (carglumic acid) treatment.

Treatment for acute high blood ammonia levels (all ages):

  • The recommended initial dose range for acute high blood ammonia levels is 100 mg/kg/day to 250 mg/kg/day.
  • Your doctor will adjust the dose based on you or your child’s blood ammonia levels and symptoms.
  • Your doctor may also administer other therapies in addition to CARBAGLU to lower you or your child’s high blood ammonia levels.

Maintenance therapy (all ages):

  • Your doctor will adjust you or your child’s dose of CARBAGLU to target a normal blood ammonia level for you or your child’s age.
  • Your doctor will divide your daily dose into 2 to 4 doses to be taken before meals or feedings.

The goal of therapy is to reduce your high blood ammonia levels to levels that are normal for your age. It’s very important to take CARBAGLU exactly as your doctor has prescribed to maintain a constant level of medicine in your body. This will prevent ammonia from building up in your blood and will help your doctor know if you are on the right dose of CARBAGLU.

How should I take CARBAGLU?

There are three different ways to take CARBAGLU orally:

For all three ways, you must mix your CARBAGLU tablet(s) in WATER only — right before you take it.

By mouth from a cup

Adults

In a cup, mix each 200 mg tablet in at least 2.5 mL of water.

Swallow the CARBAGLU mixture immediately.

Bits of the tablet may remain in the cup. To ensure that you take a complete dose, add more water to the cup and swallow the mixture immediately. Repeat as needed.

By mouth from an oral syringe

Infants

In a mixing container, mix each 200 mg tablet in 2.5 mL of water to yield a concentration of 80 mg/mL. Shake gently, but the tablet(s) do not dissolve completely.

With the oral syringe, draw up the correct amount of CARBAGLU mixture. (Discard the unused portion.)

Give your child the CARBAGLU mixture immediately.

Bits of undissolved tablet may remain in the oral syringe. To ensure that your child receives a complete dose, refill the oral syringe with a least 1-2 mL of water, and give your child the mixture immediately.

By nasogastric (NG) tube

Adults

Mix each 200 mg tablet in at least 2.5 mL of water. Shake gently, but the tablet(s) do not dissolve completely.

Administer the CARBAGLU mixture immediately through the NG tube.

Bits of the tablet may remain in the tube. Flush with more water to clear the tube.

Infants

In a mixing container, mix each 200 mg tablet in 2.5 mL of water to yield a concentration of 80 mg/mL. Shake gently, but the tablet(s) do not dissolve completely.

Draw up the correct amount of CARBAGLU mixture, and give it to the child immediately through the NG tube. (Discard the unused portion.)

Bits of the tablet may remain in the tube. Flush with more water to clear the tube.

How should I store CARBAGLU?

  • When you get a new prescription or refill of CARBAGLU, store the unopened plastic bottle containing the tablets in your refrigerator at 36°F to 46°F (2° to 8°C).

  • After you open the bottle of tablets, DO NOT refrigerate any more. Instead, store the bottle of tablets in a cool (below 86°F or 30°C), dry place.

  • Keep the container tightly closed to protect the tablets from moisture.

  • Write the date of opening on the bottle of tablets. Discard any unused tablets one month after first opening them.
    Do not use after the expiration date stated on the tablet container.

How do I get my CARBAGLU prescription?

You cannot get CARBAGLU from a local retail pharmacy. Instead, your doctor will fax your CARBAGLU prescription and other information to a specialty pharmacy called Accredo.

  • Once your prescription is verified and all paperwork is completed, Accredo will mail your CARBAGLU medication to your home in a temperature-controlled container.

  • This initial process may take some time, depending on your insurance coverage and other factors. Representatives at Accredo are committed to helping you through the process so that you can get your medicine as quickly as possible.
